How to Apply for Medicaid
Anyone 18 or older can apply for Medicaid through their local Home and Community Services (HCS) office. Locate the nearest HCS office. Learn more about what HCS staff can do for you.
Applying for Medicaid
You first need to fill out a Medicaid application. A Medicaid application can be:
- downloaded and printed off the Internet;
- picked up at a local HCS office, or mailed to you if you call HCS and ask for one.

There are three main parts to the application process, including:
- filling out the application;
- a financial review to decide if you are eligible to get Medicaid;
- an interview with an HCS case manager to help decide if and what services are needed.
Filling out the application
Fill out and return the application form. If you need help filling it out, call your local HCS office or the Senior Information and Assistance office (I & A) .
Information you will need to provide includes:
- A Social Security number;
- Proof of identification;
- Proof of income;
- Documentation of resources (such as bank statements, property tax statements, life insurance);
- Immigration or alien documents.
- Proof of citizenship if you do not receive Medicare or Supplemental Security Income (SSI).
Additional support
If you speak another language besides English or there are other things that make it difficult to understand the application process, let the HCS staff person know. Braille materials, translated written materials, large print materials, use of assisted listening devices, TTYs, or a qualified/certified interpreter or reader may be provided.
