Training Requirements & Classes - Community Instructors

The following information is designed to help new and existing community instructors get contracted to offer LTC worker courses. Community instructors are businesses or individuals wanting to teach long term care worker training in care settings throughout the community and/or home care agencies wanting to teach their own staff.
AFHs or ALFs ONLY wanting to teach their staff apply differently. Go to the AFH/ALF training application page

To provide ANY LTC worker course, use the steps and forms below.
- See Table 1 below if applying for the FIRST time.
- See Table 2 below if you are an approved Training Program and need to communicate changes.

Step 2. Apply Communicate to DSHS that you want to offer training. |
Fill out and submit a Training Program Application (TPC) form. |
Use this form to tell DSHS what courses, instructors, and curriculum you want to use.

Step 3. Contract with DSHS Contract with DSHS to offer training. |
Fill out and submit the contract intake form. |
You do not have to send in a copy of a W-9 or a Certificate of Insurance for this particular contract with DSHS. |
Step 4. Get instructors approved Identify and get approved instructors who meet the DSHS required qualifications for each course. |
Review the DSHS minimum instructor qualifications for each course you want to offer. Fill out and submit an instructor application (INS form) for Core Basic, Population Specific, Mental Health or Dementia Specialty training, or Nurse Delegation courses. Attest to the instructor qualifications in the TPC Form for Orientation, Safety Training or Continuing Education |

Step 5. Get curriculum (training materials) approved Identify or develop curriculum (training materials) to use.All training materials must be approved by DSHS. |
Decide which curriculum you want to use. This can be either curriculum (training materials):
List the curriculum name in your TPC form. If you need to get your curriculum approved, submit the materials with the required forms as listed in the next column. |

Fill out and send in a Training Program Update form if you are adding or dropping courses, instructors, and/or curriculum. |

Step 2. Get instructor changes approved Identify and get approved instructor changes (adding or removing instructors). |
Review the DSHS minimum instructor qualifications for each course you want to offer. |

Step 3. Get any new curriculum (training materials) approved Identify curriculum you are adding or dropping from your approved training program. All training materials must be approved by DSHS. |
