Basic Requirements for Participation in Roads to Community Living
Individuals eligible for this project are people with complex long-term care needs who need additional services and support to move from an institution into the community. The project will focus on the needs of older adults and individuals with developmental disabilities, physical disabilities, or mental illness.
Basic Eligibility
To qualify for Roads to Community Living, the person must:
- Be eligible for Medicaid.
- Reside in a nursing home, state hospital, or ICF-MR for at least 6 months prior to relocation.
- Require services that exceed those currently available.
Process for Participants
People who wish to participate in Roads to Community Living will complete a transition plan detailing the services and supports he or she will need to transition back into the community. This plan will be tailored to the circumstances, preferences, and needs of each individual.
This transition plan will include needed Roads to Community Living funded services and supports and existing home and community based services for the project time period (365 days from the day the person leaves the institution).
Some Roads to Community Living funded services will be one-time-only expenditures and are only available during the project time period. See some examples of potential Proposed Demonstration Services.
At the end of the 12 months, the person must transition from any Roads to Community Living funded services to available and existing home and community-based services only.
Housing Requirements
Individuals must move into a community setting which meet the following criteria:
- A home owned or leased by the person or the person’s family member.
- An apartment with individual leases, lockable access and egress, and living, sleeping, bathing, and cooking areas where the person or the person’s family member has domain and control.
- A residence in a residential setting where no more than four unrelated people live.
