Nursing AIDE Registry SCREENING
Dear Nursing Facility/Home Administrator:
I’m writing to remind you to continue to protect residents from neglect, abuse, and misappropriation of property by:
- Developing and implementing written policies and procedures that prohibit mistreatment, neglect, abuse of residents and misappropriation of property.
- Routinely screen potential nursing assistant hires by:
- Checking the Washington State Nurse Aide Registry for findings.
- Checking the Department of Health Provider Credential Search online at
https://fortress.wa.gov/doh/hpqa1/Application/Credential_Search/profile.asp. - Checking other state nurse aide registries that the facility believes may result in finding information about the individual.
To support your screening activities, a list of contact information for all state nurse aide registries is enclosed. It is also accessible online at www.cms.hhs.gov/medicaid/survey-cert/sc0546.pdf.
Staff development/human resources staff can initiate an inquiry to the Washington State Nurse Aide Registry for a prospective nursing assistant by printing a “Nurse Aide Registry Inquiry Form” (DSHS 16-193), which is available online at http://www1.dshs.wa.gov/msa/forms/. Upon the completion of the form staff will fax the form to the registry at (360) 493-2581 for processing. Registry inquiry results will be returned to the facility via fax usually within 24 hours of receipt.
