NH LICENSE RENEWAL FEE – DISCONTINUATION OF SECOND BILLING NOTICE
Dear Nursing Facility/Home Administrator:
This letter supersedes NH #2004-024, issued on September 22, 2004.
The purpose of this letter is to inform you of a change that will be occurring related to the billing notices of your license renewal fee. Currently, the Office of Financial Recovery (OFR) sends you the first and second billing notice and coupon for payment of your license renewal fee.
Effective immediately, OFR will stop sending you a second billing notice and coupon. You will receive only one billing notice and coupon for payment of your license renewal fee. This billing notice will be mailed to you on the 15 th of the month that is 60 days prior to the month your license expires. For example, if your license expires November 30, 2005, your license renewal fee is due October 15, 2005 and your billing notice and coupon will be mailed to you on August 15, 2005. You will not receive a second billing notice and coupon.
The license renewal fee payment and signed coupon continue to be mailed directly to OFR. The Business Analysis and Applications Unit (BAAU) will receive an automated notice that the renewal fee is paid. The BAAU will process the license and mail it directly to the facility.
Initial nursing home licensing and change of ownership (CHOW) licensing will continue to be handled in the BAAU. All other circumstances requiring the issuance/re-issuance of a nursing home license or modified license will now also be handled in the BAAU.
Requests for bed banking and / or bed conversion are approved by Department of Health – Certificate of Need (DOH-CON). A request for a modified license in conjunction with the DOH-CON approval request must be sent to RCS Headquarters. Include the requested effective date in your request.
