DISCONTINUATION OF PROVIDER SATISFACTION POSTCARD SURVEY

Dear Nursing Facility/Home Administrator:

Residential Care Services (RCS) initiated a provider satisfaction survey effective October 1, 2003, in an effort to collect input from providers regarding their interactions with RCS staff during RCS visits to facilities. RCS has now collected a sufficient level of information that we will cease distribution of the survey, effective July 1, 2005.

Information obtained from providers in response to the surveys has been very useful to the department as we revise procedures, train staff, and seek the most effective methods for working with the providers, families, and residents. The results of the survey will be posted on the Internet by October 1, 2005.

I appreciate all of the feedback RCS received from the providers who participated in the survey. If you have any questions, please contact your RCS Field Manager.
