Service Plan Summary Translations

In an effort to assist the field with translations of the four page Service Plan Summary document, the template has been translated into 30 different languages. This means that the headings do not need to be translated each time. This should offset some of the costs related to translating the document for client signature.

For those in the field who do not have capability to open these documents, the translation agency your local office uses can access these files on the ADSA Internet site. These translated files are in a Word document so the translation agency can enter client specific information in the document and send it back to the case manager in PDF form. To read PDF, Acrobat Reader is available for field staff to download on the CARE website.

Field staff can either fax the English hard copy versions or email the English versions of the Service Plan Summary to the translation vendor for them to enter client specific information into the translated document. Please refer the translation agency to the ADSA internet site to access the translated versions. The translation agency can email the PDF version directly to the field staff for client signature.
If the Service Plan Summary is fully developed in the field, the case manager may get verbal consent from the client using the interpreter for the assessment.
If the Service Plan Summary is completed in the office following collateral contacts, consent may be obtained over the telephone using Language Line services. This would not delay authorization of services while waiting for the translation to occur. The case manager must document in the Service Episode Record (SER) that verbal consent was given by the client.

The following translation agencies are under contract with the Department of Social and Health Services (DSHS) and can provide client specific translation requests directly from the field. Home and Community Services (HCS) and the Division of Developmental Disabilities (DDD) staff may use them now. Area Agencies on Aging (AAA) may either contract directly with these agencies or contract with any other translation agency in their area.
