SUBJECT:     CREATION OF SERVICE EPISODE RECORD FORM #02-305B

 

A new Service Episode Record (SER) form has been created.  The new form is similar to the previous one, with the exception of the title and the new form identification number, 02-305B.

 

Please use this new identification number when requesting copies of this form from the warehouse.  It is also available via the intranet.
