SUBJECT: REQUEST
FOR UPDATED ADULT DAY PROGRAMMING INFORMATION
Aging and Disability Services Administration (AASA) is in the process of updating its adult day services information.
Please complete the enclosed form. The form includes six columns:
Column 1: The Area Agency on Aging (AAA) responsible for adult day program oversight. Please review the information and make corrections if needed. Please write/type in the AAA contact person and e-mail address.
Column 2: Correct or add adult day programs that you contract with (level l) or certify (level ll) as a provider. If a provider has more than one site, include the addresses and primary contact for each site.
A program cannot be a Level 1 site unless it has a COPES contract with the local AAA. A program cannot be a Level 2 site unless it has been reviewed on an annual basis or newly surveyed for Medicaid certification on an annual basis by the AAA.
WAC 388-15-650 through 388-15-662 regulates level 1 and Level 2 services.
Column 3: List each Grantee or Parent Company for an adult day center program. An example would be the adult day program that is a part of a hospital or other company. The hospital would be the Grantee.
List the mailing address for the day services program.
Column 4: Check off if the program offers Level I services.
Column 5: Check off if the program offers Level II services.
Column 6: List each funding source available to the program (COPES, Medicaid, SCSA, 3-B, private, VA, etc).

Please return the updated form to AASA by Monday, December 3, 2001. The spreadsheet is available by e-mail for any AAA that would find it more efficient to update and edit electronically. Updated electronic spreadsheets can be e-mailed to Marietta Bobba at bobbam@dshs.wa.gov.
