Recently, the department sent you a survey questionnaire asking you about your experience during your facility?s most recent survey.? It is our goal to continue improving the licensing survey process, based upon your input.

If you have not yet completed the survey questionnaire, this is a friendly reminder that we value your input and would appreciate hearing from you.?
