NOTIFICATION OF CHANGE IN ADMINISTRATOR REQUIREMENTS

Dear Boarding Home Administrator/Licensee:

This letter contains information for boarding home licensees regarding notification of a change in administrator. Washington Administrative Code (WAC) 388-78A-2570 outlines the requirements providing notice to the Department when a boarding home administrator changes.

The Department is requesting that all boarding home licensees ensure that each new administrator completes a boarding home administrator attestation form. The form is available at http://www.aasa.dshs.wa.gov/Professional/bh.htm.

1. Prepare a written change of administrator notice to the Department.
2. Obtain a completed boarding home administrator attestation form from the new administrator.
3. Attach the completed attestation form to the written notice and mail to your local RCS field office.
