Dear Boarding Home Provider:

If you faxed Background Inquiry Applications to the DSHS Background Check Central Unit between February 28, and March 6, 2001, you must re-submit these requests.? Due to the February 28, 2001 earthquake in the Olympia area, the original faxes cannot be recovered.?

Re-submit requests by faxing to (360) 407-0506, or by mailing them to:

If you fax your inquiries, we ask that you be patient if you receive a busy signal, as we anticipate? there will be an exceptionally high volume while inquiries are being re-submitted.

Effective immediately, application forms for Nursing Homes (DSHS 09-828C), Boarding Homes (DSHS 09-828A) and Adult Family Homes, (DSHS 09-828) are now available on the internet at http://www.wa .gov/dshs/dshsforms/forms/eforms.html.?

These forms will continue to be available through the DSHS Warehouse by faxing your request to (360) 664-0597.
