PLAN FOR THE UNEXPECTED
Dear Adult Family Home Provider:
The department became aware that several adult family homes experienced an unexpected event which made the provider unable to continue to operate the home. Preplanning will ensure residents continue to receive the proper care and services needed should an unexpected event occur.
Providers Must Remember:
- A license is required to operate the home.
- The license is only valid for:
- The person named on the license.
- The address stated on the license.
- Anyone else operating the home, including a family member, is operating an unlicensed home.
Some Unexpected Events That May Occur:
- The home must be closed as quickly as possible for some reason.
- The provider leases the home and your landlord notifies you that you must move by the end of the month.
- A family emergency causes the provider to go out-of-state or country.
- The provider’s unexpected serious illness, accident or death.
Be Prepared -- You Should Plan For Unexpected Events:
- Develop written plans and procedures for what must be done if you become unable to continue to operate the home;
- Train your staff; and
- Inform your family members of the plan.
Immediately Notify the Department:
If an unexpected event occurs which makes you unable to continue to operate the home, call or have someone immediately call the department at 1-800-562-6078. This way we can all help to ensure your residents receive the necessary care and services.
