2008 AFH "Dear Provider" Letters

CHANGES TO FORMS, LETTERS AND PLAN OF CORRECTION PROCESS
Dear Adult Family Home Provider:
This is to inform that effective July 28, 2008, the following will change:
- The letters and report (statement of deficiencies or SOD) you receive after the department has completed an inspection or complaint investigation; and
- The process you use to develop a plan of correction for a cited deficiency.
You:
- Are no longer required to develop a detailed step by step plan of correction informing the department of how you intend to correct a cited deficiency;
- Must provide a date you will or have corrected each cited deficiency; and
- Must sign and date an attestation form after each cited deficiency.
The following process still applies:
The Department:
- After an inspection or complaint investigation, will send you a report if it has determined you are not in compliance with all of the licensing laws and regulations; and
- May take enforcement action based on any deficiency listed in the report.
You must:
- Within 10 calendar days after you receive the report, provide a written plan of correction:
- Indicating the date you have or will correct each cited deficiency; and
- Sign and date an attestation form after each cited deficiency.
- Begin the process of correcting each deficiency immediately;
- Complete correction within 45 days, or sooner if directed by the Department;
- Return the report to the department; and
- Have your plan approved by the department.
The Department:
- Expects all deficiencies to be corrected within the timeframe accepted by the department; and
- May inspect the home to determine if you have corrected all deficiencies.
