2008 AFH "Dear Provider" Letters

AFH LICENSE/CHANGE OF OWNERSHIP/CHANGE OF ADDRESS
Dear Adult Family Home Provider:
This letter is to remind you that you need a license to operate an adult family home, and of the requirements you must meet when changing ownership or changing location/address of the home.
It is illegal to operate an adult family home without a license.
An adult family home license is:
- Required if you provide care and services to more than one but not more than six residents in the home; and
- Not transferable and is only valid for the provider named on the license at the address listed on a license.
If applying for a change of ownership, the prospective provider must:
- Submit a new application to the department; and
- Obtain a new license.
If you change location or address of the adult family home, you must:
- Submit a new application to the department.
- Have your home inspected and approved by the:
- Local building official; and
- Department.
- Obtain a new license.
