Background Check Account Numbers and Result Letters

Dear Adult Family Home Provider:

This letter addresses two issues:

BCCU Account Numbers

The BCCU processes requests for background checks from a variety of sources, and must be able to clearly identify the organization or facility making the requests. Per the instructions contained in provider letter AFH #2005-014, you must include the prefix “A” before your license number when completing Box 4 on the DSHS 09-653 Background Authorization form.

Your Adult Family Home license number is your BCCU Account Number.

Enter “A” followed by your license number in Box 4 of the Background Authorization Form. If you do not include the prefix “A” before your license number in Box 4 of the Background Authorization form, the BCCU will not be able to process your request.

If you have previously submitted a request for a background check that did not include the prefix “A” before your license number in Box 4, please include the appropriate prefix and resubmit the Background Authorization form.

If you have not received a response from BCCU within one week of submitting your request, please e-mail the BCCU at bccuinquiry@dshs.wa.gov or call (360) 902-0299 to request the status of your background check. In order to receive an accurate and prompt response, you need to provide as much information as possible:

Please contact your Residential Care Services licensor if you need to make corrections to your mailing address or secure fax number.

BCCU Result Letters

The BCCU has revised the letters that inform you of the results of background checks. The BCCU will begin using the three revised letters (enclosed) beginning February 1, 2006.

  1. No Record of Convictions and/or Other Negative Actions Against the Applicant
    While this letter indicates no disqualifying crimes or negative actions were found
    during the background check, you remain responsible to determine the applicant’s
    suitability for employment.
  2. A Record of Convictions and/or Other Negative Actions Against the Applicant which requires a Character, Competence and Suitability Review
    This letter indicates the results of the background check found the applicant has a
    record of criminal conviction(s) or other negative actions that do not automatically disqualify the individual. However, you remain responsible to review the character,
    competence and suitability of the applicant to determine whether or not it would be appropriate to hire him or her.
  3. Convictions and or Other Negative Actions Against the Applicant are on the Secretary’s List of Disqualifying Crimes and Negative Actions
    The results of the background check reveal the applicant has a record of criminal
    conviction(s) or other negative actions that may disqualify the applicant from working in your facility. You must compare the specific conviction or negative actions against the individual with the applicable list from the Secretary and ensure you do not hire anyone who has a disqualifying record.
